Care Coordinator: NA-C/Registrar
Full-Time-NOC Shift
Summary:
The EARH Care Coordinator positions are part of a Care Coordination TEAM that provides a host of responsibilities that support all clinical departments and providers. The overall responsibility is to provide services the patient needs from and to the clinical teams. The TEAM provides 24 x 7 support and as such schedules are amendable to shifting from time to time to meet patient and clinical team support.
Duties:
- Rooming of patients and taking and recording of vitals for walk-in clinic and emergency department
- Responding to patient portal (Ask Your Doctor) questions (within scope)
- Medical record requests
- Patient referrals both inbound and outbound
- Rooming as trained
- POCT test as trained
- Patient care duties
- Other RN delegated duties
- Room turn over
- Insert and process EMS/Ambulance medical and billing records
- Registration of ER/ Walk-in patients
- Other medical document processing as assigned and trained
Minimum Requirements:
Education
- High school diploma or GED required.
- Current WA State NA-C
- CPR/BLS
